A nurse practitioner is now on call after-hours to look after residents at the Victoria Grange Aged Care and Retirement Village plus 5,000+ people aged over 65 living in a 10km radius of the village. The pilot program is designed to slash the number of hospital visits by seniors.
Previously, many of these patients would have been forced to go to the emergency room for treatment.
Developed by Australian Unity after a study trip to the US in 2015, where they came across schemes in which providers are paid to keep seniors well and active, the nurse practitioner is currently on site three days a week as well as two evenings.
Should the trial be successful, we predict that we could permanently engage two Nurse Practitioners to be on call, seven days a week, Australian Unity CEO for Independent and Assisted Living Derek McMillan said.
With CDC on its way, its a clear indication providers are innovating when it comes to providing care to their residents.
